Case Manager
Job Description

We are looking for a Case Manager to enhance the quality of care for women recovering from trauma and substance abuse disorder. Case Manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet client’s needs.


  • Coordinating and providing care that is timely, safe, effective, efficient, equitable, trauma informed and client centered
  • Handling case assignments, care plans, review client progress, and determine sobriety plan.
  • Helping clients achieve wellness, sobriety and independence
  • Facilitate multiple care aspects (care coordination, information sharing, etc.)
  • Help clients make informed decisions by acting as their advocate regarding treatment options
  • Develop effective working relations and cooperate with team and external social workers and case managers
  • Manage external collaborative relationships designed to holistically meet clients’ needs
  • Take the extra mile and interact with clients to keep track of their progress and to ensure successful completion of program
  • Record client information, accurately complete all necessary forms and produce statistical information
  • Promote quality and best practice interventions and outcomes
  • Assess and address motivational and psychosocial issues
  • Adhere to professional standards as outlined by protocols, rules and regulations
  • Develop and maintain working relationships with community agencies
  • Coordinate pre-admission interviews and assess client appropriateness for program
  • Serve in an on-call capacity on a rotating basis with other full-time staff members
  • Office management
  • Adhere to the Recovery Guiding Principles
  • Represent organization at community meetings, among callers, donors, and volunteers
  • Other duties as assigned by the Executive Director


  • BSW, CAADAC II or equivalent degree/certification
  • Minimum of two years’ experience in field of addiction, or mental health
  • Excellent knowledge of case management principles
  • Excellent communication & listening skills
  • Excellent organizational and time management skills
  • Problem solving skills and ability to multi-task
  • Compassionate with teamwork skills
  • Ability to work in high stress environment
  • Ability to work with diverse client and staff populations
  • Ability to work in group situations
  • Knowledge and familiarity with basic computer skills
